Pumps working TOO well

  • First day on pumps (started at the endo's this afternoon).

They were at 50% basal rate until 8:30 PM (when last night's half-dose of Levemir should have been out of their systems). Now that we're three or so hours into the full programmed basal rate (no bolus or food since supper at 5:30 PM) they're both low (Isaiah twice in two hours). Not drastically, but below the nighttime threshold.

I assume it's too much basal? We bumped them down to 50% again for the next several hours.

  • How much basal did they put them on?  Levemir doesn't correlate exactly to lantus or the 'logs.  It usually takes more levemir to cover- so if they put them on a basal rate equal to the number of units of levemir they were on that's likely too much (possibly significantly- 20-30%) now that their basal is 'log insulin.

  • It takes a while. I remember testing 21 times a day the first few days on the pump. Yes it was excessive!!! but it helped get basals down pretty quickly.

    Good luck!!!

  • Or it could be the excitement of starting to pump?

Yes, we did a direct conversion from Levemir and I wondered about that.

Didn't think about excitement; our day was mostly sitting (car, doctor's office, etc.) and pizza for supper so I assumed they'd run higher.  ::)

Soooo thankful for the Dexcoms, or I would have been up every hour last night!

  • I bet that's your problem.  Don't know if you're making your changes at this point, but what *I* would do is lower all basal settings by about 20% and try from there.

Annoying.  Levemir doesn't translate exactly you'd think the "professionals" would take that into consideration.  Sigh.
